Applying for Special Services at Home (SSAH) & Assistance for Children with Severe Disabilities (ACSD)

Need help in applying for SSAH and/or ACSD? Our service coordinators will help to provide support to families in completing the application(s) on line and answer any questions.

SSAH: provides funding for primary caregiver relief and related supports for activities of personal growth or development.

Eligibility: child (under 18) has a developmental and/or physical disability, needs additional support & lives at home.

ACSD: Financial support ranging from $25-$500/month and drug/dental benefit card

Eligibility: a parent or a guardian of a child who is under 18 years of age who lives at home, based on a combination of household income, size of family, severity of child’s disability, and extraordinary costs related to your child’s disability.
